These Terms of Service ("Terms") govern your use of ezpause.com and the services provided by EzPause. By booking an appointment or using our website, you agree to these Terms.
1. Description of service
EzPause provides cash-pay telehealth evaluations for the purpose of completing medical certifications related to FMLA and other medical leave (e.g., DOL Form WH-380-E or WH-380-F). Our clinician will conduct a genuine clinical video evaluation and, when clinically supported, complete the applicable certification. EzPause does not determine your eligibility for leave, and does not guarantee that any certification will result in approved leave — that determination is made solely by your employer and, in the event of a dispute, applicable government agencies.
2. Eligibility
- You must be at least 18 years old to use our services.
- You must be physically located, at the time of your video visit, in a state where our clinician is licensed to practice. EzPause currently serves Florida.
- You are responsible for providing accurate and complete information during intake and your visit.
3. Not for emergencies
EzPause is not an emergency service. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room.
4. Fees and payment
Our current fee is $175, due in full at the time of scheduling, and includes the services described on our Pricing page. Fees are subject to change; the fee in effect at the time of your booking applies to your visit.
5. Refund policy
Refunds are governed by our Refund Policy, available at ezpause.com/refund-policy and incorporated into these Terms by reference.
6. No guarantee of outcome
Certification reflects our clinician's independent medical judgment based on your evaluation. EzPause makes no representation or guarantee regarding whether your employer will approve, grant, or designate your requested leave.
7. Limitation of liability
To the fullest extent permitted by law, EzPause and its clinicians shall not be liable for indirect, incidental, or consequential damages arising from your use of our services, including any adverse employment outcome.

8. Intellectual property
All content on ezpause.com, including text, graphics, and logos, is the property of EzPause and may not be reproduced without permission.
9. Prohibited uses
- Providing false or misleading information during intake or your evaluation.
- Using our services for any unlawful purpose.
- Attempting to interfere with the security or proper functioning of our website.
10. Governing law & disputes
These Terms are governed by the laws of the State of Florida, without regard to conflict-of-law principles.
11. Changes to these terms
We may update these Terms from time to time. Continued use of our services after changes take effect constitutes acceptance of the updated Terms.
