Book your visit
Pick a time that works for you. You'll fill out a short intake beforehand, so your visit time is spent on your evaluation, not paperwork.

Generally, you need to work for a covered employer and have worked there long enough to qualify — your employer's HR team can confirm your specific eligibility. EzPause provides the medical certification; eligibility for the leave itself is determined by your employer.
FMLA covers your own serious health condition, caring for a family member with a serious health condition, and certain other qualifying reasons. Your provider will evaluate whether your specific situation meets the certification criteria during your visit.
Most patients will complete their video visit and receive their certification within 48 hours — the exact timeline depends on your provider's evaluation.
Employers can only contact us to authenticate your certification or clarify an ambiguous or illegible response — not to ask about your diagnosis or treatment. We handle up to two of these requests at no extra cost.
EzPause is a cash-pay service — we don't bill insurance directly. We provide a superbill you can submit to your insurer for possible out-of-network reimbursement.
If, after a complete evaluation, your provider determines your condition doesn't meet certification criteria, you'll be refunded your fee less a $75 evaluation fee to cover the visit. See our full refund policy below.
